How often should an SQL 2005 database be shrunk?

Based on other questions and posts that suggest that Auto_Shrink should not be turned on in SQL2005 databases, when is it okay to manually shrink the database?  
How often?
Other than disk fragmentation and performance degradatation, does manually shrinking the database cause any other problems?
Should the disk be manually defragmented on a regular basis to counteract the fragmentation that occurs during a manual database shrink?
