Thru Netbeans or j2me sdk IDE:
If you are using netbeans, then you can add these attributes like this: MidletProject->Properties->Application Descriptor->Attributes then Add the MIDlet-Icon attribute with the value as /imagename.PNG
You need to enter the following JAD attributes :

MIDlet-1: MyMidlet,/icon.PNG,
MIDlet-Jar-URL: MyMidlet.jar
MIDlet-Icon: /icon.PNG
MicroEdition-Configuration: CLDC-1.0
MIDlet-Version: 1.0.0
MIDlet-Name: MyMidlet
MIDlet-Description: MyMidletApplication
MIDlet-Vendor: MyMidlet
MicroEdition-Profile: MIDP-2.0

The 2 BOLD attributes shown above are required.

Note the following :

The icon.PNG image should be a part of the resources (should be present in the jar at the root or / or the directly the jar, not inside any folder)

The .PNG needs to be in CAPS (upper case) in the JAD as well as in the actual file name of the pic.

The picture needs to be a PNG image, not JPEG or any other format
