Creating an interactive form using MS Word 2007

How do I insert a checkbox next to my text that can be 'checked' or 'unchecked' when needed?  I am currently able to insert a 'box' or a 'box with a checkmark' but they are not interactive where I can 'check" and 'uncheck' when needed.

I'm not familiar with 2007 but it should be working in "Web layout". Go to view and change to it.

Hi, calwood28.

If I remember correctly you have to save your form as a tempalte and then create a document based on that template for the form fields to work.
