Automatic popup window form

I need to add a lightbox type popup form to a website like http://haddadink.com.
I've searching the internet to find some code but I haven't have any luck.
Does anyone know where I can find code for this?

That's the look I want but I need the code/script for a pop-up form to be filled out with that same "lightbox" effect.
