Automatic popup window form

I need to add a lightbox type popup form to a website like http://haddadink.com.
I've searching the internet to find some code but I haven't have any luck.
Does anyone know where I can find code for this?

Thanks for any ideas,
Larry

I wear a lot of hats...

"The solutions and answers provided on Experts Exchange have been extremely helpful to me over the last few years. I wear a lot of hats - Developer, Database Administrator, Help Desk, etc., so I know a lot of things but not a lot about one thing. Experts Exchange gives me answers from people who do know a lot about one thing, in a easy to use platform." -Todd S.

Thanks for your reply.
That's the look I want but I need the code/script for a pop-up form to be filled out with that same "lightbox" effect.

look the examples section. Attached is a "minimized" example:
<?xml version="1.0" encoding="UTF-8"?>
<!DOCTYPE html PUBLIC "-//W3C//DTD XHTML 1.0 Strict//EN"
 "http://www.w3.org/TR/xhtml1/DTD/xhtml1-strict.dtd">

<html xmlns="http://www.w3.org/1999/xhtml">

		<title>hielo</title><script type="text/javascript" src="http://code.jquery.com/jquery-1.4.2.min.js"></script>
		<script type="text/javascript" src='http://jquery.com/demo/thickbox/thickbox-code/thickbox-compressed.js'></script>
		<script language="JavaScript" type="text/javascript">
		$(function(){
			$('#autoLoadMe').click();
		});
		</script>
	</head>
	<body>
		<!-- since your href "points" to ajaxLogin.html, the plugin will fetch that page and use its contents in the "popup" -->
		<a id='autoLoadMe' href="ajaxLogin.html?height=85&width=250&modal=true" class="thickbox" title="Please Sign In">login (modal)</a>
	</body>
</html>
