SharePoint 2007 - Document in Unnamed

In the last 3-4 weeks, few staff are currently opening existing Word documents with the subject heading "Document in Unnamed".  Saving the document prompts a window saving it to the local draft folder and therefore will not save any latest changes back to the document library.  

This is occurring in different sites.  Anyone is experiencing the same issue?

- Internet Explorer 7
- Internet Explorer 8

Check to see if the document library is requiring you to check out documents.

It happens to document library that requires you to check it out and some that doesn't. :(
