I assume you want a child primary site which has a database in order to replicate you SMS settings!?

You have to install a sms 2003 site like your existing sms server (database, site server...). Then you have to configure addresses in both sites to enable a replication way. Then you configure your existing primary site to be a parent site and your new site to report to a parent site. (see screenshots)
