Linking Signup form in Wordpress header

I am new to php and I need to figure out how to link an image file in my header to a page in Wordpress that has a form in it?

the image file is signup.jpg and the page is signup

This assumes it's all relative and the signup page isn't a post of some sort.

The code for an image link is as LZ1 has described.

However how you add this code may be theme specific and could entail one of the following options.

1. just editing header.php but this would likely appear on every page
2. using a widget area if one is defined in the header
3. drag & drop a "leaf" if using one of the more modern themes with drag & drop design
4. using a filter on the header if using one of many theme frameworks
5. just editing something in theme options

If you have some details about the theme you are using it might be possible to give more details, though one of the better options is to start with something that has the layout you require already.
