I have pretty much cleaned this machine. I ran Malwarebytes, Combofix, Hitman Pro. All have come up clean, After this I ran spybot just to make sure I had touched all bases and found some basic spyware, nothing to really worry about.
When the service is running the machine run horrible, I can't open some Windows, like My Computer properties. Also upon shutdown I get a run32dll end now window and the computer has to be forced off.
I ran a search on Google and Experts-Exchange and found very little about this service. I can disable it in msconfig and my problems go away.
I would just like to know if this is a problem program or service and if so, does anyone know how to get it off?

The file might have got corrupted or changed by another installed program.
Try Running the following command from Start > Run: Type sfc /scannow and press Enter.
If it does find that the file has changed, you will need you Windows XP CD for the SFC (System File Check) program to replace it.

If sfc /scannow doesn't work which the previous expert suggest you then run a windows repair on this computer.
Sorry I have to appologise, I haven't actually answered your question. I don't know if that is a problem program. Disabling it in msconfig might be a good idea until you can find an answer. You might have to see if the Service exist in Services and disable it there.

I did run sfc / scannow and it completed without needing the disk, which doesn't happen very often. I did disable it prior to the post also and the computer is running fine. I was hoping to find out if anyone had seen this before, I answer here all the time and haven't seen this before. I also found very little of it elsewhere, 3 links when Googled.

Yep you're right there is hardly anything on it out there...
Something else you could try to determine what this program is:
Find the file mldocoms.exe in the c:\windows\system32 directory and then right click and select properties. Look for a version or Digital Signatures Tab. See if there is more info there that you can search on.
If that is not of much help then hopefully someone else out there has stumbled accros it that might be able to help you.

I found it in systm32 it seems to be related to a Lexmark printer, though there isn't one installed. I guess it could be a left over file.

So it should be safe to remove the service and the file....

Because the problem was a printer service and file I chose this as the solution.
