Maybe your mail file had exceeded its quota.

Also, This issue can occur if the Maximum Attachment Size option in the Mail section of the Domino Web Access tab in the Domino server's Configuration document is set to a value larger than 35GB.

Old versions of Domino had problems with foreign characters in the attachment file name.

Have you checked the maximum attachment size setting (default is 50GB).

This is a small file of 4 mb. There is no firewall.

If it works from the internal LAN but doesn't work from the internet, then it is not a Domino configuration issue, but a network issue.  Yes there is a firewall or filtering router (probably several) between your internet-connected user and your server.
