User gets "kicked out" of mailbox from all devices. Error 3005 HTTP 503

I have one user that is experiencing the following:

1 - Has a Blackberry syncing with mailbox OTA (via ATT's managed BES)
2 - Uses Microsoft Outlook 2003
3 - Messages get stuck in Outbox (doesn't matter where she composes it; OWA still keeps them in the outbox)
4 - Gets errors (operation failed) when sending calendar messages
5 - Gets "kicked out" of mailbox
     5a - Outlook displays "problem accessing your mailbox folders"
     5b - Blackberry sync stops working
     5c - Can't use OWA - Gets HTTP 503

The following is logged on Exchange's event viewer:

Event Type:      Error
Event Source:      Server ActiveSync
Event Category:      None
Event ID:      3005
User:            Domain\username
Computer:      exchangeserver
Description:
Unexpected Exchange mailbox Server error: Server: [exchangeserver] User: [username] HTTP status code: [503]. Verify that the Exchange mailbox Server is working correctly.

How do I get her back in? Simple. I restart the IIS Admin service and she works fine for about another hour or so. Then we repeat the entire process.

Any ideas??

I've done the following:
Installed applicable Microsoft updates
Followed Method 2: http://support.microsoft.com/kb/883380
Followed http://chicagotech.net/troubleshooting/activesync3.htm
Then I reversed http://chicagotech.net/troubleshooting/activesync3.htm because no one could sync at that point.

Note: She is the only person experiencing this. No one else.

Please find IIS authentication type and the SSL requirement for Exchange 2003.
1) Default Website : Annonymous & Integrated     NO SSL
2) Exadmin : Integrated                                    NO SSL
3) Exchweb : Annonymous                              NO SSL
4) Exchange: Basic                                        SSL Optional
5) RPC     : Basic                                            SSL Required
6) OMA     : Basic                                          SSL Optional
7) Public  : Basic+Integrated                         SSL Optional
8) exchange-oma : Basic & Integrated             NO SSL
9) Microsoft-Server-ActiveSync : Basic           SSL Optional
After that need to restart IIS and check it.
Optional means you can use ..

Thanks for the reply!

My settings matched yours with one exception:

1 - Exchange folder was using integrated as well as basic. I have removed integrated and restarted IIS.

After doing so, she was able to reopen the items in her outbox and actually send them!

We will see what happens over the next few hours.

I had to change that setting back. It crippled ActiveSync without integrated being checked.
