>CREATE DATABASE permission denied

seems obvious to me?!

full answer: the login you are connecting with lacks some powers.

check out which server roles that login has:

Fixed roles
The fixed server roles are applied serverwide, and there are 2 predefined server roles to allow this function:
    * SysAdmin: Any member can perform any action on the server.
    * DbCreator: Any member can create, alter, drop, and restore databases.
