Multiple Cisco 3560's showing same error : %SWITCH_QOS_TB-5-TRUST_DEVICE_LOST: cisco-phone no longer detected on port

Here is the error that shows in the switch logs:

.Apr 18 16:16:17.735 MDT: %SWITCH_QOS_TB-5-TRUST_DEVICE_DETECTED: cisco-phone detected on port Fa0/22, port trust enabled.
.Apr 18 22:28:41.867 MDT: %SWITCH_QOS_TB-5-TRUST_DEVICE_LOST: cisco-phone no longer detected on port Fa0/22, port set to untrusted.

.Apr 19 07:10:41.415 MDT: %SWITCH_QOS_TB-5-TRUST_DEVICE_DETECTED: cisco-phone detected on port Fa0/25, port trust enabled.
.Apr 19 10:01:43.402 MDT: %SWITCH_QOS_TB-5-TRUST_DEVICE_LOST: cisco-phone no longer detected on port Fa0/33, port set to untrusted.
.Apr 19 12:52:07.899 MDT: %SWITCH_QOS_TB-5-TRUST_DEVICE_DETECTED: cisco-phone detected on port Fa0/33, port trust enabled.


This error is across multiple switches. The phones(all 7912's) mysteriously reboot and then the computer connected to the phone will get a 169.254.x.x and cannot reconnect to the workstation VLAN through the phone until the computer is restarted.

The phone load is:CP7912080004SCCP080108

Here is an example of one of the ports that is having the issues configuration:

Anyone ideas?

Upgrading Call Manager did nor resolve the issue. Any more ideas anyone?

For Switches this is just an informational messages.
Upgrade the 7912's with CP7912080003SCCP070409A or the latest version of 7.x

I cannot find that firmware release from the Cisco site. Was there a reason you were suggesting this firmware? I was running on an older firmware and the problem was happening. I upgraded to the latest firmware available on the Cisco site - 8.04. I am going to try upgrading Call Manager to 7.1.3 this weekend, we are currently on 7.1.2.

I am hoping this will resolve the issue.
