Printer Setup Paper size for 3X5 Index Cards?

Need to create a report that will print onto 3X5 index cards (off a perforated roll using a dot matrix printer...please, don't ask why)...The problem is Crystal's printer setup doesn't have a paper size that will "fit".  Any suggestions?  Thank you.

You need to create a custom form. Difficult to advise with the lack of info provided. XP/Vista, open printers, right click an empty area, select Server Properties, then create a new form ......

Almost there, created a new form, Index Card 3 x 5, from Server Properties.  Restarted.  The form is NOT in the paper size drop down....how can I get it there?

It will only be available for the printer if it is within the printers range. I made such a form. It doesn't appear under my Epson Stylus, but does under Windows fax. Odd. If you have "User defined" in the paper dropdown of the printers printing preferences, you can also create it there.
