Our experience with Okidata pin-matrix printers was that the pin arrays become damaged from time to time for any number of reasons.  This can cause anything from scratching the paper, to a missing line of dots, to total failure to print.

Fortunately, this is a field-replaceable part.  Extract the pin array and swap it with a known good part, and see if this solves the problem.  If not, then the problem is likely in the driver board, and it will be cheaper to buy a used printer on ebay than it will be to find and fix the problem.

(Actually, we found it cheaper to buy used printers most of the time than chase any kind of problems in the Okis, once tech time at $85/hour was factored in.)

As an alternative:
Try restarting both the printer and the computer and check the printer cable connection. If this doesn't help, try using one of the printer emulations and try a different printer driver:

Emulation: Epson FX, IBM PPR III, (DEC®) ANSI x 3.64 2, and OKI MICROLINE

If it will pass the self test, then the problem is not in the pin array or in the driver board.

Try swapping the printer out for different one (different manufacturer), install the driver for the new printer, and see if the new printer behaves correctly.  If it does, then there is no hardware problem in the PC, and the problem is in either the Oki print driver, or in the printer logic board.
This is what i did, i print a test page and it was printing, then i install the printer in  another computer and it works fine, so i restore my computer to a previous date where the printer was working and now it is working fine

thanks
