Delete all current settings/preferences and the next time you open Photoshop it will restore the defaults
Double-click the Photoshop application icon and then immediately press Alt+Control+Shift
Or open the Preferences folder in the Library folder, and drag the Adobe Photoshop CS Settings folder to the Trash.
New Preferences files are created the next time you start Photoshop.

It did delete the preference folder but did not restore the defaults to this feature.
This can be tricky to achieve even uninstalling the software may not produce the original associations since there is always some hidden settings that saves the users preferences from projects etc.
But that maybe the only way to go, use the setup disc and try uninstalling it, the setup will detect your previous and may ask you if you want to repair it.
Delete old preferences etc.
