We're no longer able to receive external emails, however we can send out and receive internally.  This is an existing setup, as the problem just started a few days ago.  We have two sites:  each has 1 mailbox server and 1 Hub/CAS server.  Both sites are affected.  I noticed an event 15004 error on each Hub/CAS server.  It stated version buckets high.  It was 563.  Other thresholds for the queue databases were normal.  Per the following article, I increased the databasemaxsize cache to 512MB, but the problem continues.  http://msexchangeteam.com/archive/2008/05/14/448890.aspx.

I need help ASAP as this is affecting our production environment.

1. Please check whether any 1050 event is logged. If the 1050 event is logged, please temporarily disable the Transport Agent indicated in the event and check whether the issue persists.
2. Please increase the version buckets value based on the article http://technet.microsoft.com/en-us/library/bb201658.aspx

3. Please check the size of the C:\Program Files\Microsoft\Exchange Server\TransportRoles\data\Queue. If the folder is very large, please run get-queue to check how many messages stuck in the database currently. The message stuck in the all the queue should be less than 3000 and not more than 5000.
 
If many messages are stuck in the queue, please check whether the server is open relay and the anti-spam setting. If the issue cannot get improved, please further check why so many messages stuck in the queue.

There were no 1050 events in the event log...Per the Microsoft, it recommended not adjusting the threshold values in the edgetransportconfig.exe file.  Are you suggesting that I take this action?  

The size of the queue folder is 2.47 GB.  I ran the get-queue cmdlet and the total message count was about 742 messages.  The smarthost queue had 717 messages.  There is no Edge server, we have a third party filtering app.  And no anti-spam on Hub servers.
