; what is the best way to display/capture a list of questions and answers and tie it to accounts?

Hi Experts,

I have a list of questions (5 to 10 of them) I would like our customer service reps to answer for each account in What is the best way to accomplish this?

Here is an example; Stated reason for using our products within your corporate strategy? A text box would be provided for the CSR’s to enter the answer.

This is basically a survey each CSR is completing for each account they own.

I solved my issue by creating a template in MS Word and then saving it as an attachment to each account.
Question has a verified solution.
