Modify Outlook reading pane

Hey,
How can I modify Outlook Reading Pane itself ?
Such as adding button beside the Sender name or the subject..

Can I use VSTO ? How?

For example, as you can see in the image below, Can I add the smiley button?

yes you can right-click on the ribbon itself (the blank section where you have your other buttons and select customize view for 2003 or customize ribbon for 2007/2010. there you can add and remove the buttons you want or don't need. for smileys and the likes you most likely will have to download and third party application or check the micorosft office site for add-ins.
