free software for change management log

I would like to have something like a web page where IT people could log changes made to systems and when and who did it. I don't think I need anything really huge, but just something to keep track of that kind of stuff so you could look back over time to see what's changed.

Before I go writing a webpage to do this I just wanted to know if anyone knew of something like that out there already?
