Error with outlook web access

Can someone help me to answer the question below.

Question:  User reports that he can’t login to Outlook Web Access; error message is ‘User ID and password error’. He reports that he has been using the same user ID and password all along and didn’t change it. You are the only one with permission to change user password and you did not change the password. What could be wrong?

1) Has the password expired and needs to be reset?
2) Is his account locked due to some failed login attempts? If that's the case, unlock his account, reset the password.
3) is the user specifying the domain name as part of the username that he enters in OWA?  
Username:  exchangedomain\usernam
password:  users password

I will check when is the creation of the password and also the user to see which is the problem. Thanks
