4001 is quiet generic if your DB that has System Attendant mailbox is dismounted or you have removed it .... i mean the default store ??

Can you share some screenshot or more information when it started and if any changes were made ??

9564 - Do you have Teaming ??
It looks more like Networking issue ... Is there some stale Computer object in AD for any DC or Exchange server not used ??
Can you check the NIC options - IPv6 enabled ??
Also how many NIC's do you have ... is the Binding order fine ??
