Infopath template to add in Outlook.

Hi,

Infopath template to add in Outlook.

Can anyone help me with a template that i have just these

Name
Dropdown that will have 50 names

These are just 2 things that i need to get details from the users.

Use Infopath to create the form, you will need a place to submit the data too, sharepoint, sql database, file share.  Then publish the form via email.
Users will get the form and fill out, when they submitt the data will go where you selected it based on the submit data connection you create.

Can you help with a sample template please..
A dropdown with some data I want to export the selections to excel. So one i receive emails need to export
