What's the best CRM software package today for a medium size enterprise of about 1,000 people?

Curious on what great CRM packages people have found in the current marketplace.  Anything notable worth recommending for a company considering on replacing Siebel?  We're thinking of moving forward with SAP since we have an SAP backed, but concerned of it being more than we need for CRM and not having some of the "best of class" usability features that we can see in SalesForce.  Is there anything else we can recommend?

i second veedar's answer. it might not be the best for you, but its for sure very very good and also opensource with lots of support,

if you want it free and also opensource you might want to have a look at
CiviCRM

if you are into a small business and dont mind expending a little i would suggest Salesnexus

We're a fairly large (1,000+) employee healthcare supply chain hardware and software organization.  We are considering implementing an SAP CRM module if that gives you an idea of the resources we have for the system.  While some open source products look appealing I don't think they'd be an option for us because we have a small IT department.  We currently are using an older Siebel enterprise solution which is very outdated.  The MS dynamics solution proposed is interesting but it only looks like an Outlook add-in.  Am I correct?  It seems to be a rather cumbersome solution lacking a developed UI.  We'd be looking for something similar to SaleForce.com.  Am I missing something with MS Dynamics?  We are a MS shop so it would assimilate nicely with our other systems.   Does it have a Knowledge Base module?
The MS Dynamics CRM solution is not an Outlook add-in, it is a CRM platform based on web services in its own right. The web interface that is shipped with the application sits on top of the CRM platform and it is an option for you to customize it or indeed ignore it and write your own front-end. With regard to Outlook, there is indeed an optional (no extra charge) CRM Client for Outlook that a) makes CRM available within Outlook and b) integrates some record types such as appointments, tasks and contacts with Outlook folders.

MS Dynamics CRM does have a knowledge base module but it is fairly simplistic.  However, the beauty of CRM is that it can be extended. I suspect that if you went for CRM you will need to spend quite a bit of time customizing it to meet your needs but on the other hand at least you can change it whereas with other products it may be a bit harder or not possible to change.

While good recommendations I would have liked to receive more packages where company's have had good experiences.
