Print Comments in the Footer or Endnote Section of Each Page

In Word 2003 I know I can print Comments as Balloons on the side of a page and I can also print a "List of Markup".  Unfortunately I need to print the Comments for each page at the bottom of the page as a footer or endnote.

Can anyone provide detailed instructions for this.

There's a macro at http://www.vbaexpress.com/kb/getarticle.php?kb_id=183 that will extract comment text and place as footnotes.
