Capture card recommendation for Windows 7

Hi All

Can anyone recommend a video capture card that would be appropriate for capturing relatively low resolution video i.e. 352 * 288 NTSC from composite or S-VIDEO feed?

Ideal product would be in the price range $500-600 USD.

Other requirements are a simple GUI for recording and saving a file, and seeing the input feed on screen while capturing.

For PCI or PCI Express, need clarification on the card. Will get that ASAP - though certainly PCI (non-express) for now.
