SQL Cluster 2nd Instance Installation

We have a windows 2008 R2 cluster running only a single default instance of SQL 2008, I want to install a 2nd instance of SQL 2008.  My questions are:

1)  I know the 2nd instance needs it's own disk group resource, is the witness (quorum) disk shared between instances or does each instance need it's own quorum and data disk, so 2 disks per instance?

2)  Does it matter that the first instance is a default instance and not a named instance?

3)  I was unable to find much info on installing the 2nd instance, are there any good links out there that detail the install?

4)  Sorry to ask this one but are there any gotchas I should keep in mind?
