What is a simple popup date picker for an ASP project?

I am doing an ASP project where the user enters information in a form and the information is sent to an Access database. On one of the form fields (example month), I want to include a simple popup calendar picker that will give me the same date format and then send the entry into the proper field in the Access database.

I have read a couple of previous entries on popup calendars on the knowledge base, but I am looking for a simple one, and ways to code it.

Suggestions? I have included the form code below. Thanks.

This is a great help and will be a good start. Two things:

1.) The day only goes to 30. O to 30. What if you months with 31 days?

2.) If you can later help me with the original request of the date picker, that would be great. Beggars can't be choosy. I wouldn't need all the fields of year, month, day. I would go to date.

For some reason, the Year, Month and Day do not enter into the database. I am attaching in the code field, the entire page. All the other fields in the database collect information, but not the Year, Month and Day fields.
