hp 4250 TN form print

Ineed to configure an HP 4250TN printer to print correctly on the attached form; as you can see on the top is missing about 2 lines, I appreciate all the help I can get.

What application is printing this? Is it a DOS program, or does it use a Windows driver?

If it's a Windows driver, can you check things like Page Setup or Printer Setup and make sure the top margin is set correctly?

The application is eclinicalworks but this only generates a word document so the application is MS-Word and yes it uses a windows driver.

Check the page settings in Word, under Page Setup, *not* from printer properties at print time.
