Check your Apache error logs. One reason could be too many concurrent connections to your server. If you can find the word MaxClients in the error logs, then that is the case.

How do I check on these logs?

They are usually located in /var/log/apache. Grep those files for MaxClients and/or browse them with any file viewer for indications of other errors.
