To transfer the domain you need to get an auth code (from the web hosting company). With this auth code you can go to any other company and initiate a transfer for your domain, using just the domain name and the auth code. Then the new company will take care of the domain transfer.

Hope this helps.

Well that could be an issue. You will need the access to the control panel or the auth code to be able to transfer the domain. By using one of those the current registrar gives his approval for the domain transfer. So you'll have to treat them nicely until you get what you need from them :-~

You are quite correct. After (or during) the move of the website you create an A-record for your domain which points to the new ip address and a CNAME record which point the www to the A-record (atleast that's how I do it). So something like:

If you get access to the current control panel you can always have a look at how it is set up there.
