Dreamweaver 4, templates, moving nested index.html to primary

I downloaded a free template for use in DW CS4.  I see the index.html nested a couple of folders down.  I can open the template, and edit it, but of course when I publish to my web server, my index.html is still nested.

How do I make that index.html nested down two folders deep become my default home page so that when my users type www.mydomain.com, my site will load the nested index file?

Why not move the index.html up to the web root and fix the paths to the images and includes?  

That's a helluva lot easier than setting up a Rube Goldberg-esque system of RewriteRules that basically break your filesystem?
