how should I include the file into my MS VS folder?

I had VS 2008 Express and now, the better, VS 2010 Express. Neither have MFC/ATL so I wasn't able to provide you with step-by-step instructions. Sorry about that.

I take it that you are not using the Express edition, so that you do have MFC/ATL.
I may try to figure out how to install this into Express edition.
