Cisco CME conference issue

I have a user that has issues with creating a conference. He will hit the conference button, putting a call on hold, and then call the other party. But when he then hits the confernce button again, the screen just says "Unable to conference". This is CME 4.0.

I have read that this could be an issue with codec mismatches, but I am not sure where to start looking in the CLI. What commands can I use to find out if this is the case?

Also what commands do I use to to recored logs for a phone call where he tries to do this, so i can get more information on the error?

what parties are being conferenced? i.e. three internal VoIP users or, two VoIP users + one PSTN user, or one VoIP user connecting with two PSTN users?
Does this only happen for one user (all other users can conference ok)?  or for one particular person you are connecting to the conference?

Sorry on being so vague. I wastn sure exactly what the issue was. I figured it out though. This was a new system that was setup by someone else. I found that there were no ephone-dn's assigned to ad-hoc conferencing. I created them and that fixed the issue.
