Spreadsheet for tracking Beta Testers for Office 2010

I am trying to create a spreadsheet for Beta Users in various different departments across companies. Running out of ideas for column names to get the most detail tracking info. Any suggestions?

Name, Company, Department, Commencement_Date, Age_group, State, City
