OWA 2003 login twice problem

I have 3 Exchange servers:
master server A (OWA for LAN users)
member server B (users do not login to OWA directly here)
frontend server C - in the DMZ (OWA for external users)

- OWA have to login twice when mailbox is located in member server B.
- Only need to login once if mailbox is in master server A
- The same if login from the frontend server C, twice if mailbox is in member server B and once if mailbox is in master server A

All are Exchange 2003 SP2
POP/SMTP, RPC/HTTPs are working fine from all directions.

Anyone know how to fix the double login on OWA?

It could be several things, but first, I would make sure that Forms-Based Authentication is not enabled for server B (you can't tell from the above images).  I would also enable Integrated Auth (as well as Basic) on teh Exchange VDir on server B.  Make any changes in Exchange System Manager, because the settings from there will periodically overwrite the IIS Manager settings.
