How can I export all of the security permissions for all the sites and access in the SharePoint  farm?  Perhaps to excel or is there another simple way / tool  to do this?

As mentioned in the previous comment the Quest tool or writing .NET code or PowerShell script to walk the entire site structure (including lists).  Sorry but there is nothing out of the box for doing it.

PS: Because the site structure is hierarchical spreadsheets don't do a great job of showing permissions.
