Create form in WSS 3

There is a form for request stationery.
I want to use WSS3 to automate the process.
Could someone tell me how to do it step by step? Such as create the form and then the workflow.
I am new to sharepoint or WSS3.
Thanks a lot.

Staff apply the request form, then the request will be send to a mailbox.

You can't use InfoPath on WSS 3.0, (http://www.bizsupportonline.net/blog/2008/12/how-to-enable-infopath-forms-services-in-wss-v3/) so the only way to create this form is to create a custom application page. (http://msdn.microsoft.com/en-us/library/bb418732(v=office.12).aspx)

That page will be your form and will create a new item which then is linked to a workflow.
