Form Creator to replace excel worksheet

Hi all,

I have an excel worksheet which I want to replace.
Basically it's used as a form that is filled in and then printed.
Does anyone know of some software that I could use to create some forms, with text areas, checkboxes etc that can be filled in and then printed out please?

Microsoft Word has form fill capabilities.
