How to add picture into new form in WSS 3?

I want to add some pictures into New Item Form.
I created a column with picture type, but it can not show the image when create new item.
It need user to input URL..

How can i do it just the attached screen-shot?

customize your new item form using sharepoint designer and add a picture
here is a way
