What is good software for designing templates for PDF's?

I have to design a number of different templates that will ultimately be flattened PDF's pre-filled with data in them.  

These templates are corporate documents (things like share registers, corporate minutes etc).  I'm wanting to create nice looking templates (things like boxes with rounded corners, nice tables etc).

What software would be best suitable for this?  It needs to output a PDF that I can then bring into Acrobat and create form fields in etc?

You can use Adobe Photoshop to design the templates and then use the dialog "Save as..." to export those images as PDF files. Then it is easy to use them in Adobe Acrobat
