recovery password cisco router 3945

Dear Cisco ,

I buy new Cisco router 3945 first time booting I put user and password cisco after I make reboot and save I did not change anything now I cannot login router said wronge username and password , before username and password is cisco
How I make recovery password for this router !!

The new imeages only once eneble cisco / cisco, you need to rebbot the router and you able to login, after that you need to create new username:

username xxxx privilege 15 sec yyyyy
ena sec zzzzz

i put interface SM-ES2-24 in router 3945 when router be on doesn’t show interfaces of slot (SM-ES2-24) . can you tell me what is solution .
why cannot read this slot
