In Microsoft Office Word, how do I create the following templates?

I am trying to create a business ledger with the categories at the top in upward-slanting open columns. I have this in hard copy form from an office supply store, but I need to know how to create it in Word.

Also, sometimes I go into a store and someone advertises something with little vertical tear-off sheets with their contact number. How do I create the tear-off section, essentially turing the columns in a vertical direction? Thanks in advance.

Oh, I have MS Word 2007.
Insert text box in document. The right-click the text box and go to format section. Go to 3D rotation and rotate the Z axis by whatever degree you want.

McOZ, you are correct - Word can't do slanting table cells, only vertical and horizontal. I was confusing it with Excel, which makes more sense for a ledger, anyway. Thanks for the help.
