There are several templates built into Office 2010 (or at least downloadable from MS site)

What sort of form are you looking for?

There are several options.
Simpliest basic form,

create the form on your screen.

Save as TEMPLATE when you save.
First, you have to enable the Developer tab.
(File>Options>Customize Ribbon>Check Developer in Main Tabs)
then go to Developer Tab, click on Visual Basic button
On the Visual Basic Windows, right click on the Microsoft Excel Objects Foldern, Chose Insert>User Form.
Then you can start editing the form.
