In SSMS, right click 'databases', select 'restore database'.  Enter your new database name, choose 'from device' then input the full path to your bak file.  Click ok and your restore will create the new database.  

If you want to control the location of the mdf and ldf files, you will have to select the 'options' page and modify the paths there.  

After the restore completes you will have to create your logins and map those to the existing (de-orphan) or create new users in your database.
