Modify contact page

I have attached the contact page for my theme:
Currently, it looks like this: http://www.elegantthemes.com/preview/PureType/contact/

Please tell me how I can modify the .php file so that "Name", "Email address", "Subject" and "Message" appear as labels, rather than inside the textfields.
