Now problem is I have -is having other fields ( shipDate,Run,chk1,chk2,chk3,chk4, CVSKey be added into table -with the serial number. and is it possible?

The simplest way is to base the form on your table, then add controls to your form that are based on the various table fields. If you "bind" them, Access will manage the data work for you.

Easiest if those additional fields are already in the table. Can you explain why they are not?

See the attached database. I've updated the Add Record code so it will add all fields on the form to the table as long as the control name on the form matches a field name in the table.
