PHP javascript error appears in email

We have a request app, that will send out a email confirmation email once the request has been submitted.  We have one client, that is getting the attached error when they open up the email confirmation.  I thought there was something up with the function, but when I submitted the request so I could get the email, I did not get the error and none of the other 52 clients that we have are getting this.

HOw do I determine what is causing this for this one client?

Found the issue - for those who run across the same problem:

Upon opening an email in my Lotus Notes account, I receive this JavaScript error message, "JavaScript error was encountered while loading a page. Error on line 674: ReferenceError: NOF_Menu is not defined." Do you know why I'm receiving this message?
This is typical behavior of Lotus Notes prior to version 8 (and even some in 8) for HTML-formatted email messages. Lotus Notes has never rendered email with tags and code in it well. I'm assuming your email messages are being stored in the sender's format (not Notes rich-text), so there is not much you can usually do here.
