Access 2007: Query as record source for form shows 1/1 records and any data entered is as a new entry rather than working with previous values from combo box selection

Hello, I have a form called frmDecisionTree that some users here have helped me with, but it has a problem.  The form is intended to work with qryDecisionTree as the record source and already have entries present.  The user will use a combo box to select the appropriate record and then move through the check boxes on the form.  However, the form always shows 1/1 records (although the query which is the record source has 10 or so entries) and when I select a record from the combo box control it does not move to that item.  Could anyone please take a look and let me know how to fix this issue?

    Sometimes it is impossible to find reason :(
    I've created new form (frmDecisionTree1) and it is working. Filtering on Call number was also added

    I didn't read your previous question - about field in query. But it ([Reviewed Item]) was also corrected in uploaded example.
