outlook adding second copy of attachment

I have a strange outlook problem.  When one of my users adds a word attachment, sometimes it will add a second copy of the file, which is corrupt and cannot be opened.  This is happening randomly.

Make sure the user is actually adding the word document and NOT the temp file created by word when a doc is open for editiing. I have seen this cause all kinds of issues like that
Is this happening for all users?
Check in OWA if this is happening...

If there is an outlook issue then recreate outlook profile and check.

This normally means USER ERROR...

How are they attaching the doc? From a new email? From inside word using send?
Do they ALWAYS do it the same way? Is the file in use when they try to attach it?
