Dynamics CRM 4.0, SSRS Activities Report, Phone Call, Appointment Fields

I am writing a SSRS Activities report to import into CRM on Email, Phone Call and Appointment activities.  Can anyone tell me where to find the Recipient field for a Phone call activity and the Required field for an Appointment activity.
